Chief Hospital Corpsman Samuel J. Warner, a native of Clear Lake, IA, joined the Navy on June 16, 2004 and attended boot camp at Recruit Training Command, Great Lakes. He graduated from Hospital Corpsman “A” School, Great Lakes, in December of 2004.
His duty stations have included Marine Corps Recruit Depot Parris Island, 3d Battalion 8th Marines, Helicopter Antisubmarine Squadron FIVE “Nightdippers” (HS-5), 2d Battalion 24th Marines, NOSC Des Moines, Marine Special Operations Command Marine Raider Training Center, Coastal Riverine Squadron FOUR, and the Office of Naval Intelligence. Chief Warner is currently serving as the Senior Enlisted Leader of Fleet Surgical Team SIX.
He has deployed to various overseas operations, exercises, and contingencies in the 2nd, 4th, 5th and 6th Fleet areas of responsibility.
He is also a graduate of Field Medical Service School, Aerospace Medicine School, Naval Aircrew Candidate School, Navy Search and Rescue, Survival Evasion Resistance Escape (SERE), Navy Instructor Training, U.S. Army Flight Medic, U. S. Army Airborne, Plans Operations and Medical Intelligence (POMI), and Executive Medical Department Enlisted Course. He holds Lean Six Sigma Black Belt certification and has his Associates Degree in Health Science from Kaplan University and graduated Magna Cum Laude with Bachelors in Health Science, Health and Wellness from Purdue University in 2019.
Additionally, he has earned his Fleet Marine Force Warfare Specialist, Naval Aircrewman, Naval Parachutist, Expeditionary Warfare Specialist, and Information Warfare Specialist designations.
His personal awards include Navy/Marine Corps Commendation Medal x3, Joint Service Achievement Medal, Navy/Marine Corps Achievement Medal x6, Combat Action Ribbon and various unit and campaign awards
